First Time Visit
- Bring your insurance and a photo ID
- Optional: Download the form below and fill out before visit. (If this isn't filled out, printed, and brought to the office, then it will need to be filled out when you arrive.)
- Arrive early (about 10-15 minutes) to fill out additional paperwork in office.
During The Visit
- Initial consultation with a possible x-ray
- Scheduling for actual adjustment (adjustments do not happen during first consultation visit)
